Clinical Case Study: When Ten Years of Constipation Finally Began to Ease

A Seven-Day Naturopathy Journey at Nirvana

Sometimes a health problem becomes so familiar that we stop treating it as a problem.

For Mrs. Leelawati, constipation had been part of everyday life for almost ten years. Difficult bowel movements, bloating, acidity and a burning sensation in the chest had gradually become something she had learned to manage. Alongside this, she had been living with Rheumatoid Arthritis for nearly four years, with joint stiffness and discomfort adding to her daily challenges.

At 41, she decided that instead of continuing to simply manage the symptoms, she wanted to spend some time focusing on her overall health. This brought her to Nirvana Naturopathy & Retreat, Igatpuri, Nashik, for a seven-day residential naturopathy programme from 06 September to 12 September 2026.

Her programme was conducted under the clinical supervision of Dr. Roshni Bala and qualified BNYS doctors at Nirvana.

Understanding the Complete Picture

The initial approach was not simply, “How can we make the bowel move?”

After ten years of constipation, there was a much bigger picture to understand. Her digestive complaints included bloating, acidity and burning heartburn, while her history of Rheumatoid Arthritis meant that her physical comfort and joint stiffness also needed consideration.

A personalised programme combining natural hydrotherapy, mud therapy, abdominal applications, therapeutic massage, dietary modification and lifestyle guidance was therefore planned.

Her constipation symptoms were also assessed using the Patient Assessment of Constipation Symptoms (PAC-SYM) questionnaire. At admission, her recorded score was 21, providing a baseline for monitoring her progress.

Hydrotherapy and Mud Therapy

Hydrotherapy became one of the important elements of her naturopathy programme. Selected therapies such as hip baths, underwater massage and aqua-based cleansing procedures were administered under professional supervision.

Mud therapy and selected abdominal applications, including castor oil packs, gastrohepatic packs and cold packs, were also incorporated according to her individual treatment plan.

For her joint-related discomfort, selected therapies such as Nirvana Rejuvenation Massage, deep tissue massage and herbal potali applications were included according to her clinical requirements and tolerance.

The idea was never that one therapy would solve everything. The therapies were combined with appropriate food, rest, hydration and a regular daily routine.

A Simple Herbal Drink

As part of the dietary wellness routine, a mild cumin-coriander-fennel infusion may be used when considered suitable by the BNYS doctor.

Simple recipe: Take ½ teaspoon cumin seeds, ½ teaspoon coriander seeds and ½ teaspoon fennel seeds. Add them to approximately 500 ml of water, simmer gently for a few minutes, allow the preparation to cool to a comfortably warm temperature and strain before drinking.

It can be taken in small quantities during the day rather than as a concentrated kadha.

For a patient with acidity and burning heartburn, the important point is moderation. Strongly spiced or highly concentrated herbal drinks are not automatically better and may aggravate symptoms in some individuals. The drink should therefore be used only when clinically appropriate and well tolerated.

Food Became Part of the Treatment

Diet was another important part of Leelawati’s programme.

Her meals were kept simple, fresh and predominantly Sattvik, with suitable cooked vegetables, moong dal, light khichdi, vegetable soups and appropriately selected seasonal fruits.

Because she had both constipation and bloating, fibre was approached gradually rather than suddenly increasing large quantities of raw salads or bran. Depending on tolerance, foods such as papaya, pear or soaked raisins could be included as part of the dietary plan.

Adequate hydration, regular meal timings and mindful eating were encouraged. Highly processed foods, deep-fried preparations, excessive chilli and very spicy meals were reduced, particularly where they appeared to aggravate acidity or digestive discomfort.

The emphasis was simple: give the digestive system consistency rather than constantly changing remedies.

The Clinical Change

At discharge on 12 September 2026, her PAC-SYM score had reduced from 21 to 3.

During the seven-day programme, Mrs. Leelawati also reported considerable relief from bloating, acidity, burning heartburn and hard stools. She reported improved joint flexibility and a reduction in arthritic discomfort during her stay.

Her metabolic and cardiovascular parameters remained stable under clinical observation.

The PAC-SYM change provides an objective measurement from this particular treatment episode, while her reported symptoms describe her personal experience.

More Than Seven Days

The real challenge begins after leaving a naturopathy retreat.

Leelawati was advised to continue the recommended dietary and lifestyle practices at home, including appropriate hydration, regular meals, mindful eating and consistency in her daily routine.

Her case does not suggest that every patient with chronic constipation or Rheumatoid Arthritis will experience the same outcome. It represents one individual clinical observation following a supervised seven-day programme.
